HRT for Perimenopause: Why Hormone Therapy Isn’t the Whole Answer
I'm going to say something that might surprise you: I don't believe HRT is the solution for perimenopause.
Let me be clear about what I mean, because this isn't an anti-HRT take.
Can hormone replacement therapy be incredibly beneficial? Absolutely. HRT is an important and highly effective tool for hot flashes, night sweats, sleep disruption, miserable periods, vaginal changes, and other not-so-fun symptoms tied to shifting hormone levels.
But somewhere along the way, the conversation around perimenopause got reduced to a formula:
You're in your late 30s or 40s. You don't feel like yourself. Therefore, it must be perimenopause. And the answer must be HRT.
Every Symptom Isn't Automatically Perimenopause
Open social media and you'll find virtually every symptom under the sun being attributed to perimenopause: fatigue, anxiety, weight gain, bloating, brain fog, insomnia, low libido, hair loss, blood sugar crashes. The list goes on.
Hormonal shifts can absolutely contribute to all of these symptoms. But hormonal shifts are not the only thing happening. And adding hormones will not automatically resolve them.
This is where I believe the current conversation is failing women.
Most women starting HRT don't have a clear picture of what's actually happening in their bodies. Are they tracking ovulation? Are their symptoms driven by declining hormones, fluctuating hormones, or issues with processing and clearing hormones? What's going on with their thyroid, blood sugar, cortisol, digestion, nutrient status, and sleep?
Because HRT is not a magic bullet. It is a tool.
And when it becomes the entire solution, it distracts us from the foundational imbalances that are making women feel like their bodies are fighting against them during this transition.
What We See Constantly in Clinical Practice
Women entering perimenopause are already running on empty.
Skipping breakfast. Drinking coffee on an empty stomach. Under-eating carbs and calories. Pushing through intense workouts on too little sleep.
Blood sugar is unstable. Mineral and nutrient reserves are depleted. The nervous system is on high alert. Digestion is sluggish. They're sleeping six hours, caring for everyone around them, carrying an enormous mental load, and wondering why their bodies suddenly feel so much less resilient.
Then perimenopausal hormone fluctuations arrive and expose every crack in that foundation.
What HRT Can't Do
HRT may help address a genuine hormonal need. But it cannot do the rest of the work:
It cannot replace adequate nourishment
It cannot stabilize your blood sugar
It cannot restore depleted magnesium or B vitamins
It cannot build a consistent sleep-wake rhythm
It cannot compensate for chronic under-eating, overtraining, or a nervous system that never gets to power down
When these foundations stay unsupported, women often see limited improvement on HRT, and assume they just haven't found the right dose yet. Sometimes that's true. And sometimes the symptom was never exclusively about estrogen or progesterone in the first place.
Where the Real Work Starts
This is why my first priority for women approaching perimenopause is strengthening their foundation. That means:
Eating enough, with appropriate amounts of protein, fiber, carbohydrates, and healthy fats to meet their personal needs
Building balanced meals that support steady blood sugar
Identifying and correcting nutrient deficiencies
Supporting digestion and the pathways involved in hormone metabolism
Protecting sleep and optimizing circadian rhythms
Adjusting exercise so it builds resilience instead of adding more stress
Creating real space for the nervous system to downregulate
These strategies are not a replacement for medically appropriate HRT. They are the groundwork that helps your body navigate hormonal changes more effectively, with or without it.
So No, I'm Not Anti-HRT
I am against women being told that every symptom after 38 is perimenopause, and that the only meaningful answer is a prescription.
You deserve a more complete conversation. You deserve to understand what your body is actually communicating. And if you do choose HRT, you deserve comprehensive support that helps you respond to it well and get the most out of it.
Build the foundation first. Then let the tools do their job.
